Terms of Use
Last updated: November 7, 2026
These Terms of Use apply to the Elevera Health website, eleverahealth.com, and to the telehealth services you book through it. They’re offered by Elevera Health PLLC, doing business as Elevera Health (“we” or “us”). By using this website or booking a visit, you agree to these Terms and to our Privacy Policy. The Elevera Health mobile app has its own App Terms of Use.
1. Website content is not medical advice
- Everything on this website, including text, images, articles, prices and testimonials, is for general information only. It isn’t a substitute for professional medical advice, diagnosis or treatment.
- Using this website, filling out a form, signing up for emails or messaging us doesn’t make you a patient or create a provider–patient relationship. That relationship begins only after you complete your intake and your first visit with one of our providers.
- Always ask your provider about a medical condition or treatment, and never ignore professional medical advice or delay getting it because of something you read on this site.
- In an emergency, call 911 or go to the nearest emergency room. Our website, forms, email and messaging aren’t monitored for emergencies.
2. Who can use our services
- You must be 18 or older.
- We provide care by telehealth to patients who are physically located in a state where our providers are licensed (currently Virginia, Maryland, Washington, DC and West Virginia) at the time of each visit.
- You agree to give us complete and accurate information about your identity, location, health history and medications.
3. Our telehealth services
- Your provider decides, using their professional judgment, whether a treatment is right for you. A prescription is never guaranteed, and your provider may decline, change or stop a treatment for medical reasons.
- Results vary from person to person. We don’t promise any specific result, including any amount of weight loss. Testimonials reflect individual experiences.
- Some medications we prescribe are compounded by licensed pharmacies for individual patients. Compounded medications are not FDA-approved and haven’t been reviewed by the FDA for safety or effectiveness.
- Telehealth has limits. Your provider may recommend an in-person exam, lab work, or care from another clinician, urgent care or an emergency room.
4. Fees and payment
- Our visits are self-pay. We don’t bill insurance for visits. Insurance may cover lab work and some medications, depending on your plan.
- New patients pay a one-time $100 initial consultation fee. Current prices for visits, programs and medications are on our price sheet. Prices may change; the price that applies is the one in effect when you receive the service.
- Consultations, visits and medications must be paid in full at the time of service.
- All fees for consultations, visits and medications are non-refundable, except where the law requires a refund. If there’s a problem with a medication order, such as a pharmacy or shipping error, contact us and we’ll work with you and the pharmacy to resolve it.
- If you join an optional membership plan, its price and terms are explained when you sign up.
5. Cancellations and missed appointments
- Please cancel or reschedule at least 24 hours before your appointment.
- If you cancel less than 24 hours before your appointment, or miss it without cancelling, a $100 cancellation fee applies.
- To cancel or reschedule, use our patient portal, message us in Spruce or call (703) 952-3366.
6. How we communicate with you
- We may contact you by phone, text message, email, our patient portal or Spruce about your appointments, care, prescriptions and billing.
- We send marketing emails only if you’ve chosen to receive them. You can unsubscribe at any time with the link in any email, and doing so won’t affect your care.
- Email and text messages aren’t always secure. For questions about your health, use Spruce or the patient portal.
7. Acceptable use of this website
Don’t misuse this website. That includes trying to get into parts of the site or our systems you’re not allowed to access, interfering with the site, sending spam or malicious code through our forms, scraping or copying our content in bulk, or posting comments that are unlawful, abusive or contain someone else’s private information.
8. Our content
The content on this website, including text, graphics, photos, logos and design, belongs to Elevera Health or its licensors and is protected by copyright and trademark law. You may view and print pages for your own personal, non-commercial use. Any other use requires our written permission.
9. Other websites and services
Our website links to services run by other companies, such as our patient portal and scheduling system (Charm Health), Spruce, pharmacies, app stores, review sites and social media. Those services have their own terms and privacy policies, and we’re not responsible for their content or practices.
10. Disclaimers
To the extent the law allows, this website and its content are provided “as is” and “as available”, without warranties of any kind, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose and non-infringement. We work to keep information current, but medical knowledge, prices and availability change, and the site may contain errors or be temporarily unavailable.
11. Limitation of liability
To the extent the law allows, we are not liable for indirect, incidental, special, consequential or punitive damages arising from your use of this website. Our total liability for any claim arising from your use of this website is limited to $100. This section applies to your use of the website; it does not limit any liability that can’t be limited by law, including liability for the professional medical care our providers give you.
12. Governing law and disputes
These Terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict-of-law rules. Disputes will be resolved in the state or federal courts located in Loudoun County, Virginia, unless the law of your state requires otherwise. Before filing a claim, please contact us so we can try to resolve it informally.
13. Changes to these Terms
We may update these Terms from time to time. The “Last updated” date at the top shows when they last changed. Changes apply from that date forward and don’t change fees for services you’ve already received.
14. General
If any part of these Terms is found unenforceable, the rest stays in effect. If we don’t enforce a right, we haven’t waived it. These Terms work alongside your consent to treatment and the intake forms you complete as a patient; if they conflict, your signed patient forms control for your medical care.
