Elevera Health mobile app

Effective date: October 26, 2026 · Version: 2026-10-26

This section covers the Elevera Health app for iPhone and Android (the “App”).

Who we are

The App is offered by Elevera Health PLLC (doing business as Elevera Health), a telehealth medical practice, and is built and published for Elevera Health by T and T Solutions LLC. App store listings show T and T Solutions LLC as the developer. In this section, “we” and “us” mean Elevera Health PLLC and T and T Solutions LLC.

Mailing address: 1019B Edwards Ferry Rd NE #1060, Leesburg, VA 20176. Questions: email us or (703) 952-3366.

The App is a personal companion for tracking your health and wellness between visits. It is not your medical record: that is kept in Elevera Health’s electronic health record system. The App is also not a way to contact your care team. For care, message your provider in Spruce or call the office.

Apple Health and Health Connect data is never used for advertising or marketing, and is never shared with third parties except the service providers described below.

Collected automatically

  • Device and app information: a push-notification token, an app-installation identifier, the device platform and the app version. Used to deliver Elevera Health news notifications and to show update prompts.
  • App integrity check (Apple App Attest / Google Play Integrity). Used to confirm requests come from the genuine App.
  • Security and service logs: our cloud provider keeps access logs, which can include IP addresses and timestamps, to protect the service and investigate problems.
  • Anonymous usage counts: daily totals, such as how many times a recipe was opened today. These counts contain no name, account ID, device ID, location or health details. You can turn them off in the App’s settings.

We do not:

  • use advertising, ad networks or ad tracking
  • use third-party analytics or crash-reporting tools
  • sell your information
  • use your health information for marketing

How information is used

  • To provide the App’s features: tracking, reminders (scheduled on your phone), charts, recipes, workouts and programs.
  • To keep your account and data secure, prevent abuse, and meet legal obligations.
  • To send practice news by push notification (for example, new office hours). These messages never include your health information, and you can turn them off at any time.

Elevera Health staff manage practice content (recipes, workouts, announcements). Staff cannot view your health logs or photos in the App.

Who processes your information

  • T and T Solutions LLC builds and runs the App and its backend for Elevera Health PLLC, under a Business Associate Agreement.
  • Google Cloud / Firebase hosts the App’s backend: sign-in, database, photo storage and server functions. These services are covered by a Business Associate Agreement with Google Cloud. Data is stored in the United States and encrypted in transit and at rest.
  • Apple and Google deliver push notifications and provide Sign in with Apple, Google sign-in, and the App Attest / Play Integrity checks. Notification content never includes health information.

We share information beyond this only if you ask us to, or if the law requires it.

If you are an Elevera Health patient, your medical records and the information you share with your care team through our clinical systems are protected under HIPAA, as described in our Notice of Privacy Practices, which you can request from the office.

Your choices and rights

  • Delete your account: in the App, go to Me → Settings → Delete my account. This permanently deletes your account, logs and photos from our systems. Backup copies are removed automatically within 14 days. You can also email us. See also Delete your app account.
  • Get a copy of your data: use Export my data in the App’s settings where available, or email us.
  • Correct your data: you can view and edit everything you entered in the App, or email us.
  • Turn features on or off in the App’s settings: push notifications, anonymous usage counts, cycle tracking (and its cloud sync), Apple Health / Health Connect access, and the app lock.
  • Delete cycle data at any time with one tap in cycle-tracking settings.
  • Depending on where you live, you may have more rights under state law. Contact us to use them; we won’t discriminate against you for doing so.

Retention

We keep your App data while your account exists. Deleting your account deletes it right away from the live systems and within 14 days from backups. Anonymous usage counts are kept for about 13 months, and security and audit logs for about 13 months. Records of staff actions are kept as required for compliance.

Security

Our safeguards include:

  • encryption in transit and at rest
  • access controls so that only you can reach your data
  • two-step verification for staff
  • integrity checks so that only the genuine App can reach our servers
  • audit logging and daily backups

No system is perfectly secure. If a breach affects your information, we will notify you as the law requires.

Children

The App is for adults 18 and older. We do not knowingly collect information from children.

Changes

We will update this section as the App changes. We will ask you to review material changes in the App before you continue.
